Approximately 10% of patients with chest trauma present lesions of the heart. This manuscript presents two cases of cardiac involvement with different causal kinematics, reviewing their clinical presentation, application of diagnostic methods, treatment and prognosis. Cardiac trauma is associated with a very high degree of mortality, considered in the IX century as untreatable; its diagnosis should be made in a timely manner; the high degree of clinical suspicion and the mechanism of trauma, could guide us to the correct diagnosis in an early manner. Penetrating cardiac injury is a life-threatening condition that usually requires emergency surgical intervention; among the causes, civil violence has been pointed out, most of them being fatal. About 20% are admitted alive to the hospital and it is necessary to develop emergency measures, creating trauma centers with established protocols in these cases. The survival rate is related to the extent, the mechanism, the need for emergency surgery and the presence of complex cardiac lesions.
