However, the rare occurrence of malignant neoplasms such as keratinizing squamous cell carcinoma (SCC), related to chronic inflammation induced by foreign bodies, represents an emerging clinical concern. Case report: A 53-year-old female with a complex surgical history and a recent diagnosis of gastric cancer presented with severe weight loss, enterocutaneous fistulas, and purulent secretions. Diagnostic evaluation confirmed an infiltrating keratinizing squamous cell carcinoma associated with a previously placed prosthetic mesh. The patient underwent subtotal gastrectomy, resection of the compromised mesh, and Roux-en-Y reconstruction. The postoperative course was complicated by hypovolemic shock and bile leakage, which required management in the intensive care unit and further surgical intervention. This case illustrates an extremely rare but clinically significant complication, highlighting the possible association between prosthesis-induced chronic inflammation and carcinogenesis. Conclusion: This report adds evidence to the limited literature on SCC associated with prosthetic mesh use and reinforces the importance of long-term clinical surveillance in patients with prosthetic implants. It also emphasizes the need for a multidisciplinary approach for the timely diagnosis and management of these unusual complications.
2025-05-12T00:00:00+00:00 Copyright (c) 2025 Killkana Salud y Bienestar https://killkana.ucacue.edu.ec/index.php/killcana_salud/article/view/1650 Duodenal follicular lymphoma: cause or incidental finding of dyspepsia. Most cases are diagnosed incidentally during endoscopic procedures. Objective: To describe a case of duodenal follicular lymphoma diagnosed in the context of nonspecific dyspeptic symptoms. Clinical case: A 54-year-old female patient with no relevant medical history presented with intermittent abdominal pain and distension of several years' duration. Due to the persistence of symptoms, an upper gastrointestinal video endoscopy was performed, which identified a mucosa with multiple whitish nodular elevations, with a granular appearance, distributed diffusely, with a cobblestone mucosal pattern and without evident alterations in the vascular pattern. The biopsy was compatible with follicular lymphoma, later confirmed by immunohistochemistry. Conclusion: Primary duodenal follicular lymphoma is a rare entity with a favorable prognosis. Early detection allows for a conservative approach in most cases. The correlation between endoscopic findings and histopathological studies is essential to avoid misdiagnoses and define appropriate management.
2025-05-12T00:00:00+00:00 Copyright (c) 2025 Killkana Salud y Bienestar https://killkana.ucacue.edu.ec/index.php/killcana_salud/article/view/1649 Ultrasound imaging findings in women with infertility: a literature review 2025-06-16T15:10:15+00:00 Byron Salomón González Jara byrons.gonzalez@ucuenca.edu.ec María Belén Calva Albarracín belucalva10@gmail.com José Patricio Beltrán jose.beltran@ucuenca.edu.ec
Introduction: Infertility represents a global health problem, affecting a considerable percentage of couples of reproductive age, generating various physical, emotional, and economic consequences. In this context, ultrasound emerges as a fundamental diagnostic tool due to its accessibility, non-invasive nature, and low cost. Objective: The main objective is to analyze the most relevant imaging findings obtained through ultrasound, in order to consolidate the existing evidence on the utility of this technique in the evaluation of female infertility. Methodology: A comprehensive literature review of 34 scientific articles published between the years 2020-2024 was carried out. The search was conducted in the Pubmed, Scopus, and Google Scholar databases, using a search strategy that combined health descriptors, such as: "Fertility female", "Ultrasonography", "Ecografía". Results: The literature review highlights the crucial role of ultrasound, particularly the transvaginal approach, in the evaluation of the anatomy and structures of the female reproductive system. It allows for the detection of various alterations such as ovarian cysts, uterine malformations, and pelvic adhesions. Additionally, it facilitates the monitoring of ovarian response and endometrial vascularization during assisted fertility treatments. Conclusion: Transvaginal ultrasound is established as a key diagnostic tool in the management of female infertility. Its value lies in its ability to provide relevant diagnostic and prognostic information, allowing for a personalized and comprehensive approach in the management of this condition. In case of limitations, it requires supplementation with other diagnostic techniques.
2025-05-12T00:00:00+00:00 Copyright (c) 2025 Killkana Salud y Bienestar https://killkana.ucacue.edu.ec/index.php/killcana_salud/article/view/1591 Therapeutic and industrial potential of plant enzymes: papain, bromelain, and ficin. The body does not become ill in the abstract; each symptom expresses living conditions, family dynamics, work tensions, and structural inequalities. From a humanistic perspective, it is proposed that medical practice should transcend biomedical diagnosis to incorporate narratives, silences, and cultural knowledge that broaden the understanding of the health-illness process. Likewise, ethics and the doctor-patient relationship are highlighted as essential pillars of contemporary clinical practice, reaffirming medicine as a deeply human discipline where listening to the patient also implies listening to their history and context.
2025-05-12T00:00:00+00:00 Copyright (c) 2025 Killkana Salud y Bienestar
